It was in the birth of her fourth child that the homeopath Wilma Kuhllmann, 67, lived a completely unexpected experience. "I was squatting and the baby came out very fast. As it passed through my vagina, I had the happiness of having an orgasm without even knowing it existed," she recalls.

This happened in 1980, in his first experience of having a child at home . After that, three more came, but the experience of pleasure never repeated itself again.

And it is not surprising: to have an orgasm in childbirth, according to gynecologists interviewed by the Universa report, it is possible, but something very rare to happen by many factors.

"From an anatomical and physiological point of view, they are the same organs, hormones and mechanisms of orgasm in sexual intercourse. So, it's quite possible to happen," says Carmen Simone Grilo Diniz, an obstetrician and professor at the Faculty of Public Health at USP (University of Sao Paulo). Wagner Hernandes, also an obstetrician at USP, agrees: "Possible, it is, but in 20 years of childbirth, I've never seen it, the pain is very intense, it's very difficult for a woman to stay calm and relax for an orgasm."

Childbirth should be a positive experience

Carmen Diniz, however, disagrees with him on this point. She argues that labor pain can be large or small, depending on the woman, but points out that there is another pain that is caused by the conditions under which gestation and delivery take place and directly influence the possibility of pleasure at the moment.

"The physical pain of childbirth is different from that caused by the use of synthetic hormones, to be immobilized in a non-physiological position, to have the vagina cut, the belly pressed, to suffer verbal abuses ... Then the degree of physical and emotional discomfort to which women undergo can negate any possibility of a positive birth experience, "he says. Yes, she's talking about obstetric violence. 

For Wagner Hernandes, talking about childbirth is complicated. "It is such a rare thing, so difficult to measure, because it is not objective. For us, doctors, the important thing is to bring the mother and baby healthy, if in the meantime she can enjoy it better," she says.

Point of which Carmen also disagrees. "Since safe care is the starting point, as you, the professional, can you do to make that experience the best it can be?"

In line with this reasoning, WHO released a manual of recommendations for childbirth care in February for a positive delivery experience in which it provides guidelines for medical practice. Respect for maternal choice is a recurrent orientation of the document.

Helping the well-being of women during pregnancy and childbirth is exactly the role of doulas. And for some of them, having an orgasm at the time of the child's birth is possible. "I have not had it, but I know of people who have had it," said Doula and psychotherapist Bruna Morais, who will give a workshop on the subject this week in São Paulo. 

For her, the first point for pleasure to have space at this time is to understand that pregnancy is part of female sexuality. "It was a whole sexual process that culminated in gestation and it is something sexual, not medical," he says.

Kalu Gonçalves, also doula, agrees with her and believes that "preparing for a pleasurable birth should be the focus of all pregnant women and all the environments that welcome these women."

She herself experienced the experience in her first pregnancy. "When the baby slid, I really felt a very deep orgasm." It was this that made her decide to study to be a doula and thus help other women to improve their labor experiences.

The essential point, according to them and also according to Carmen, is for the woman to have a quiet, more conscious and active gestation and, at the moment of giving birth, have a welcoming environment and assistance that helps to relax.

"Unfortunately, professionals are trained to think that childbirth is an unbearable, unworthy, disgusting and dangerous experience," explains Carmen.

The expert also stresses that pain is part of childbirth, but anatomically pleasure can also do. She recalls that the clitoris is not only a spot seen on the outside of the vagina, but a complex organ that can be stimulated during the baby's passage.

Would not the search for orgasm be an oppression anymore?

Both doulas and gynecologists report that few times they saw a woman having an orgasm during childbirth. If this is so rare, then why talk about it? Is not it just another way to create frustration for pregnant women?

"You go to a sexual relationship and you do not know if you will enjoy it or not, but it is important to know that the possibility of enjoyment exists," says Bruna Morais. 
The question, according to the professionals, is not that the woman has orgasm as an objective, but as a possibility. "If the focus is for the woman to seek a pleasurable birth and to face it with all the dynamics necessary for a good sex, the birth becomes a revolution because the woman assumes the protagonism of her body, of its autonomy. , which in the case is the doctor, who sees it as the center and serves it as it wishes. "The environment is tranquil, and pleasure is in the whole process, and orgasm is something that can happen," explains Kalu.
